The UK railway is entering its biggest structural shift in decades. As Network Rail and train operating companies move towards a more integrated system under Great British Railways, the way the industry operates will change fundamentally. Bringing track and train closer together creates the opportunity for a more joined up, effective railway. But it also means aligning larger, more distributed teams across a single organisation.
The consistency challenge
In a large, complex organisation, incident investigation is rarely performed by a single central team. It is carried out by people at different levels, across different sites, with different levels of training and experience. What holds that together, or fails to, is methodology.
When methodology is consistent, a near-miss investigated in the North West is recorded, coded, and analysed in the same way as one in the South East. Root causes are categorised using the same taxonomy. Contributing factors are identified through the same structured process. That consistency means the data is comparable, and patterns can be spotted. Leadership can make decisions based on evidence rather than assumption.
When methodology diverges, that comparability disappears. You end up with investigation reports that cannot easily be aggregated, root cause data that reflects individual interpretations rather than systemic patterns, and a learning function that operates in silos. The individual investigations may be thorough, but the organisation is not learning at the level it needs to.
Safety leaders across the industry are already dealing with this challenge, and the pace of reform is only adding to that pressure.
What GBR reform means for investigation practice
The Great British Railways vision, and the work being done to develop it, is explicitly built around integration. Joining up infrastructure and operations, reducing fragmentation and creating the conditions for a railway that functions as a coherent system rather than a collection of separate businesses.
Digitalisation is central to that vision. The language coming from rail leadership is consistent: data-driven decision-making, shared platforms, joined-up working across the network. That is the direction of travel, and it is the right one.
For investigation practice, this creates both an opportunity and an expectation. The opportunity is to build, for the first time, a genuinely unified picture of safety performance across the railway, not just within a single TOC or region, but across the whole system. If investigation data is captured consistently, using a common methodology and a shared taxonomy, it becomes possible to spot the failure patterns that no individual organisation could see on its own.
The expectation, increasingly reflected in ORR's approach to oversight and in RAIB recommendations, is that rail organisations can demonstrate not just that they investigate incidents, but that their investigations drive learning and prevent recurrence. That is a harder bar to meet when investigation methodology varies across teams.
Methodology as infrastructure
There is a tendency to treat investigation methodology as a secondary concern, something that sits behind the immediate operational priorities of running a safe railway. In practice, it is foundational to them.
A structured methodology does several things that ad hoc investigation cannot. It ensures that the same questions are asked every time, regardless of who is leading the investigation. It provides a common language for root causes that makes data comparable across teams and over time. It creates an audit trail that demonstrates rigour to regulators and internal stakeholders. And it gives investigators a framework that holds under pressure, when the temptation to reach for the nearest explanation rather than the real one is at its strongest.
For a devolved railway trying to operate as an integrated system, a shared methodology would help distributed teams build a shared picture of what is actually going wrong across the organisation.
Digitalisation as the enabler
A consistent methodology requires more than a document or form that gets filled; it requires a digital environment in which the process is embedded, where investigators are guided through a structured approach as a matter of course, where data is captured in a standardised format, and where the output feeds into something that can spot patterns across the whole organisation.
This is where the digitalisation agenda and the investigation consistency agenda converge. The platforms that support investigation work in rail are not just efficiency tools; they are how methodology gets applied consistently at scale, and how the learning from thousands of individual investigations becomes visible to the people who need to act on it.
For rail organisations working through the GBR transition, the direction of travel is clear. The more important question is whether the platforms they choose were built for safety-critical investigation, or adapted from general-purpose systems that were never designed for it.
